Support That Comes to You. Adventures That Wait for You.
At Care 4 Community, we believe independence starts at home but shouldn’t end there. Whether you need a helping hand with your morning routine or a companion to explore the best of Melbourne, our team is right beside you.
Our In-Home & Community Support services are designed to give you the freedom to live safely in your own space while staying connected to the world around you.
1. Assistance with Daily Living (In-Home Support)
Your home is your sanctuary. We help you keep it that way.
We provide flexible support delivered by qualified staff who respect your privacy, your routine, and your rules. Whether you need 24/7 complex care or just a few hours of help each week, we tailor our services to your NDIS goals.
Personal Care & Wellbeing:
- Morning & Evening Routines: Assistance with getting out of bed, showering, grooming, and dressing to start your day with confidence.
- Medication Management: Our staff follow strict “Six R’s” protocols (Right Medication, Dose, Time, Route, Person, Documentation) to ensure your health is managed safely.
- Nursing Care: Professional support for wound care, continence management, and chronic health monitoring (diabetes, respiratory) right in your bedroom.
Domestic & Household Assistance:
- Meal Preparation: We help you plan healthy menus, shop for groceries, and cook nutritious meals that you enjoy.
- Housekeeping: Assistance with laundry, dishwashing, cleaning, and maintaining a hygienic living environment.
- Skill Building: We don’t just do it for you; we teach you how to do it yourself (where possible) to build your independence.
2. Community & Social Participation
Life is better when we’re connected.
Social isolation can be a barrier to happiness. Our Community Access programs are designed to help NDIS participants in Cranbourne, Frankston, and Clyde build friendships, learn new skills, and feel a true sense of belonging.
How We Help You Engage:
- Social Outings: Accompanied transport to cafes, movies, parks, and local events.
- Skill Development: Support to attend classes (art, cooking, computers) or join sports clubs (swimming, gym).
- Capacity Building: Learning how to use public transport, handle money, and interact with the community confidently.
- Group Activities: Join our arranged group days to meet peers with similar interests in a safe, supported setting.
How Funding Works
These supports are typically funded under the Core Supports category of your NDIS plan:
- Assistance with Daily Life: Covers your in-home personal care and domestic help.
- Assistance with Social & Community Participation: Covers the support worker’s time to take you out (ticket costs/entry fees are usually paid by you).
3. In-Home Nursing Care
In-home nursing involves a wide range of clinical services administered by Registered Nurses (RNs) or Enrolled Nurses (ENs) directly in your home. Unlike standard support work, this service is focused on medical monitoring and treatment.
Our program is designed to ensure you can continue your personal relationships with family and friends in familiar surroundings, rather than feeling isolated in a hospital environment.
We offer flexible support tailored to you:
- Hourly Care:For specific treatments like injections or wound dressing.
- 24/7 Care: Around-the-clock support for high-intensity needs.
- Scheduled Visits: Regular check-ins to monitor vital signs and health progress.
4. Household Tasks & Domestic Assistance
Keeping a home running smoothly takes time and energy-energy that you might prefer to spend on your hobbies, your family, or your health. At Care 4 Community, we provide reliable Household Tasks & Domestic Assistance to ensure your home remains a safe, comfortable sanctuary.
Whether you need a one-off deep clean or regular weekly visits, our friendly support workers are here to lighten the load so you can focus on living your best life.
