Expert Care for Complex Needs.
Standard support isn’t always enough. When you are managing complex medical conditions or recovering from a significant accident, you need a team with higher qualifications and deeper experience.
At Care 4 Community, we are more than just an NDIS provider. We are a specialized care organization equipped to handle high-intensity needs, Transport Accident Commission (TAC) claims, and WorkSafe recovery plans across Cranbourne, Frankston, and Melbourne.
1. TAC (Transport Accident Commission) Services
Recovering from a road accident? We are here to help.
If you have been injured in a transport accident, your road to recovery is unique. We are a registered TAC provider, meaning we can deliver the specific support services funded by your TAC claim.
- Personal Care: Assistance with showering and dressing while you recover mobility.
- Home Services: Keeping your home clean and safe while you focus on rehabilitation.
- Community Access: Transport and support to get you to rehab appointments and back into the community.
- Therapy Support: Working alongside your physio or OT to ensure you are doing your exercises safely at home.
2. WorkSafe Support
Injured at work? Let’s get you back on track.
Workplace injuries can be physically and mentally draining. We work with authorized WorkSafe agents to provide the in-home care you need to recover.
- Attendant Care: Professional support workers to assist with daily living tasks.
- Post-Hospital Care: ensuring your transition from hospital to home is safe and supported.
- Return to Health: We focus on your capacity, helping you regain the independence needed to eventually return to work.
3. NDIS High Intensity Supports
